It depends on your definition of clean but northern Europe has certainly nosedived in recent years.
People cut back on dental costs and energy bills by showering less. The current government advice is to shower 5 minutes every 3 days. Especially for women it is kind of this empowering 'feminist' thing not to give a fuck about what people think about the state of your hygiene. There is an uptick in rashes and whatnot. I think less than < 50% wash their hands after visiting the bathroom. 

The pretty boys from Turkey with their white shining teeth and perfectly groomed beards can't offset that.  

Some countries like Brazil and South Korea might go overboard with plastic surgery but they won't just leave a big lump on their chin like the Europeans do.

As someone who experienced 5 years long relationship with a narcissist woman, all I can say narcissist people will never change. Never.

At one point he mentions that his wife has hyperemesis gravidarum. Hyperemesis gravidarum is a condition during pregnancy that causes extreme vomiting and nausea, as well as weight loss.

So she's suffering all of this while pregnant with their second kid, and he boasts:

Cried reading through this. I am really proud of you for getting to this point. I've not suffered in a situation with the stakes that you have, but I've been close to people like this and know what they're capable of.

You're on the right trajectory. The advice the people around you have been giving you is correct. Plant a firm wall between you and her, and help the kids climb over it to you. Give your kids a safe place to express their pain. And just keep going. Really truly, I know how hard it must have been to get to this point. And the grief you feel is not just over what you lost but over what you may be realizing you never really had.

- Hubby registers the child thus 'confirming' it is his
- Wife treats hubby like shit, not so secretly cheats with chad
- Hubby can't handle it anymore, divorces wife
- Wife and chad enjoy double income and tell kids their father is a POS and shower them with gifts paid for by their father

Sometimes even worse, for example accusing the hubby of beating the wife (with boyfriend as witness ).
In other cases the parties reach some kind of understanding to pretend to stay together until the kids are 12 or so to keep up appearances but the other route is much more financially beneficial for the mother. It doesn't matter if she marries someone else, the person that registered the child is legally the father and financially responsible.

The "emotional" it doesn't matter that it is a donor, you are still the 'father' that likely made him file the paperwork, is going to fuck this guy up for the rest of his miserable life.
