That's the kind of stuff that happens when the guidelines are too vague. I wouldn't be surprised at all if itch.io presented the rules exactly as they were told, with no specification about the content being pornographic or not.
It's not rare to see vague censorship rules. I.e. for about 10 years or so FPS games in Germany would have the ragdoll physics removed because publishers assumed this was a censorship criteria, only eventually it turned out to be a misunderstanding and they didn't have to do all that to begin with.
But if you ask me that's the kind of misunderstanding the people behind the censorship are actually quite happy with, rather have people be overly cautious and self censor instead of having them all on the edge the line of what is legal.
Will a game with a bear sex scene like Baldurs Gate 3 get censored? If the answer is "maybe" then publishers won't risk adding it in the first place
